Registration Fee $125 for anyone who has formerly participated in Artstarts. Registration Fee $200 for anyone who has not participated in Artstarts.

If registering as an Artstarts participant, please enter at least one production or camp that you participated in at Artstarts. *

HEALTH INFORMATION AND GENERAL RELEASE

Please list any of the participants known medical conditions that you believe the Directors should be aware of, or enter NONE. *

The undersigned enters this agreement with Artstarts Company, the producer of The Greatest Theater Camp, to be held virtually/on line. I have been informed and understand that Artstarts Company and the Directors may photograph or record participants during the production of this camp. I grant Artstarts Company the right to use my child's name, likeness, image, voice, appearance and performance, hereinafter referred to as the "Product", and the right to broadcast, exhibit, market, sell, display, and to post the Product to Artstarts Company's website, Facebook, and other social media outlets, and otherwise distribute the Product from the production of The Greatest Theater Camp.

The participant, or if the participant is a minor, the participant's parent or legal guardian, agrees to hold Artstarts Company and the Directors harmless from any liability for loss or injury which may occur during the production of The Greatest Theater Camp. Photography or videoing camp is STRICTLY prohibited by anyone other than Artstarts Company and the Directors.

I confirm that I have a right to enter this agreement on behalf of the child whose name is indicated below, that I am not restricted by any commitment to third parties, and that Producer has no financial commitment or obligation to me as a result of this agreement.

I have read the foregoing and understand and agree to its terms. I have read The Greatest Theater Camp information sheet and understand the camp requirements and policies and agree to abide by those terms.
